Frequently Asked Questions

What are the criteria for admission into the Life Adjustment Program? 

  • Must have a medically documented diagnosis of a brain injury.
  • Must be over the age of 21.
  • Medically stable (not requiring extensive direct care).
  • Must have an accepted payor source (Brain Injury waiver, long term care, Medicaid, workers compensation, or private pay).
  • Have or can obtain Social Security Income/Social Security Disability Income for supplemental costs for program’s room & board.
  • Be able to physically navigate large, open spaces on campus with minimal support.
  • Ability to participate in program, personal care, and medical management.
  • Demonstrate appropriate conduct for campus including the lack of substance abuse, criminal activity, and violence.
  • Able to participate in resident driven care plan and Life Adjustment Program policies and procedures.
  • Able to perform direct care services without maximum supports.

    What does the admission process look like? 

    • Completed referral packets are reviewed by our team. Once the review is complete if the resident meets requirements on referral packet a campus tour and four hour stay/meet and greet is held. If culture fit is appropriate an official assessment will follow. Once all needed steps are complete resident will be formally admit into the Life Adjustment Program.

    Do I need a doctor? 

    • Yes, we can support with physician referrals.

    Will I have a roommate? 

    • We offer 2 or 3 bedroom apartments with a shared living space and kitchen area. All residents have their own private bedroom and will share common areas of the apartment with roommate(s).
