Life Adjustment Program
Hilltop’s Life Adjustment Program (LAP), established in 1985, is a long-term supportive housing community for adults with brain injuries. We create individualized, multi-disciplinary care plans that focus on physical, emotional, and social wellbeing. A variety of daily activities, a beautiful campus to explore, and kind staff encourage residents to participate and engage in the program. With an average stay of 12.5 years, we’re proud to be a place where people want to work, live, and come together.

We believe in putting “adults first” by focusing on two things – how we treat one another, and how we treat the people we serve.

We Focus On

- Working off of a transparent, resident-driven, multi-disciplinary comprehensive care plan with measurable and achievable goals.
- Supporting individuals with navigating their goals and interests.
We support residents by modeling the “5 Things”:
- Setting boundaries – generosity towards others while staying true to ourselves
- Being predictable – we do what we say we’re going to do
- Being consistent – providing consistency through boundaries, values, actions, and beliefs
- Acting with kindness – through compassion and empathy
- Communicating with transparency – through open and honest communication

- Case management
- Protective oversight
- Assistance with ADL as needed
