What are the criteria for admission into the Life Adjustment Program?

To be considered for admission, individuals must meet the following criteria:

  • Have a medically documented diagnosis of a brain injury
  • Be over the age of 21
  • Be medically stable and not require extensive direct care
  • Have an accepted payor source (Brain Injury Waiver, Long-Term Care, Medicaid, Workers’ Compensation, or Private Pay)
  • Have or be eligible to obtain Colorado LTC Social Security Income (SSI) or Social Security Disability Income (SSDI) for room and board costs
  • Be able to physically navigate large, open spaces on campus with minimal support
  • Be able to participate in the program, personal care, and medical management
  • Demonstrate appropriate conduct, including no substance abuse, criminal activity, or violence
  • Be able to participate in a resident-driven care plan and follow Life Adjustment Program policies and procedures
  • Be capable of performing direct care tasks without maximum support

What does the admission process look like?

Because every individual is unique, each admissions journey at the Life Adjustment Program is thoughtfully tailored. Once we receive a completed referral packet, our team will review it. If the applicant meets the criteria, we’ll schedule an assessment and campus tour and meet-and-greet. Following completion of all steps, and review by senior leadership, the resident may then be formally admitted into the Life Adjustment Program

Do I need a doctor?

Yes. You will need a physician, but we can help with referrals if needed.

Will I have a roommate?

Yes. We offer two- and three-bedroom apartments with shared kitchenets, restrooms, and living spaces. Each resident has their own private bedroom and shares common areas with roommate(s).
