Supportive Living with Purpose.

Established in 1985, Hilltop’s Life Adjustment Program (LAP) is the only supportive living community for adults with brain injuries on the Western Slope—and is home to up to 90 individuals. We specialize in creating personalized, multi-disciplinary care plans that support each resident’s physical, emotional, and social wellbeing. Our vibrant campus is full of life, with daily activities, compassionate staff, and a culture rooted in fun, acceptance, and meaningful connection. With an average stay of 12.5 years, LAP isn’t just a place to live—it’s a community where you have a sense of belonging.

Our daily activities are designed to support wellbeing, encourage connection, and promote independence. These include:

  • Cooking classes
  • Meals and snacks
  • Medication administration
  • Case management
  • Protective oversight
  • Assistance with activities of daily living (ADLs), as needed

Some activities may have an additional cost.
